All posterior teeth have pot holes in them. But in medical terms they are not called pot holes. They are known as pit & fissures.

Brushing and flossing are the best ways to help prevent cavities, but it’s not always easy to clean every nook and cranny of your teeth – especially those back teeth you use to chew (called molars). Molars are rough, uneven and a favourite place for leftover food and cavity-causing bacteria to hide.
Cavities (dental caries) begin to develop within these pits and fissures. Sticky and retentive foods along with cavity causing bacteria and saliva lead to production of acids. These acids further break down the tooth, create holes and lead to cavity formation.

Sealants are a thin protective coating that adheres to the chewing surface of your back teeth. There is no substitute for brushing and flossing, but they can keep cavities from forming and may even stop early stages of decay from becoming a full-blown cavity.

According to the CDC, “school-age children without sealants have almost three times more cavities than children with sealants.”

How long do sealants last?
The whole purpose of a sealant is to protect vulnerable teeth surfaces while the teeth are newly erupting. Studies have claimed that good retention is seen up to five years after their placement. The resin-based sealants showed the best retention rates.
Cost-Effectiveness of Dental Sealants?
The most important question to be asked here is whether sealants prevent the formation of dental cavities or not?
Dental sealants do seem to be a COST-EFFECTIVE intervention; sealing permanent molars reduces the total cost by preventing the need for more expensive and invasive restorative treatment especially in children at a high risk of developing cavities (caries).
Caries risk is assessed using indicators such as low socio-economic status, previous caries experience, sugar consumption between meals, the presence of active white spot lesions, and low salivary flow.
